Form AOS302 contains three forms: the Affidavit of Non-Military Status, Affidavit of Service by Mail, and Affidavit of Default. The Affidavit of Non-Military Status is used by the Petitioner to affirm that the Respondent is not in the military. The Affidavit of Service by Mail is used by the Petitioner to affirm that they served the Findings of Fact, Conclusions of Law, Order for Judgment and Judgment and Decree on Respondent to the Respondent through the mail. The Affidavit of Default is used by the Petitioner to schedule a default hearing if they have not received a response from the Respondent within 30 days of the date the Summons and Petition was served.

This form supersedes F414 in cases involving Dissolutions without children.
