Notice to Obligor of Medical Support Enforcement

This form is used when one party has been ordered to provide proof that they have obtained health and dental insurance for specified minor children, and they did not provide such proof within 30 days of the order. If after another 15 day they have not provided written proof of the insurance coverage, the court order will be sent directly to the noncomplying party's employer, and the premium for the insurance coverage will be withheld from the party's paycheck.
